For Applicants With NBI Clearances Issued 2014 Onwards

If you have the Clearance issued by the National Bureau of Investigation onward 2014 and want to renew your Clearance, and do not want to make any changes in personal information like name, date of birth, place of birth, or civil status. The options are:

Option: 01

Follow these steps for NBI clearance renewal from abroad. 
  • Authorize any person to apply for Clearance on your behalf in the home country
  • Provide the copy or original old NBI certificate
  • Two 2×2 picture with white background
  • Passport date page picture
  • Consent or authorization letter
  • Pay the 200PHP by the representative

Option: 02

Send the application directly to NBI Manila on the below address:

IRD- Mailed Clearance Section, National Bureau of Investigation, Taft Avenue, Ermita 1000 Manila, Philippines.

And Provide these documents with application.

  • Provide the copy or original old NBI certificate
  • The self-addressed returned envelope should be stamped and used to deliver the NBI Clearance from the Philippines to the country in which you are currently living.
  • The payment receipt of the fee (Fee is 200PHP)

How to Apply for Consularization of NBI Clearance Form No. 5 at The Philippine Consulate of the Canada

Form No. 5 is a fingerprint card form used to obtain the clearance certificate from the NBI office, The Consulate of the Philippines in Canada consularizes this form. To apply for Form No.5 there are different ways i.e.

  1. For those who do have not any NBI clearance and He/She got the Clearance before 2014
    • Book an appointment on www.vancouverpcg.org under consularization of Form No.5
    • Or send the Request letter to the Consulate on Philippine Consulate General 999 Canada Place, Suite 660, Vancouver, BC V6C 3E1 address for Form No.05
    • Bill the form on both sites with the correct information
    • Visit any nearest consulate of Phillippines or police station for a Fingerprint impression
  2. For those applying in person at the Consulate:
    • Book an appointment and submit the following documents
    • Two 2×2 pictures with a white background
    • Passport date page photocopy
    • Cash payment of CAD$36.25
    • If you want to get the Form No.5 on the same day appointment then you have to pay the Extra CAD$14.50 or otherwise, you will get it after two days of processing time.
  3. For those applying by Email for Form No.5
    • Email the following documents in Email to the consulate i.e.
      • Two 2×2 white background photographs
      • Passport data page photocopy
      • Payment receipt of CAD$36.25 in bank draft or money order
      • Complete contact details i.e. Email, address, and phone number
      • Self-addressed return envelope
